6. All of the above in one game

Cannot recall in my lifetime ... anyone else remember it happening

2, 3 & 4 occurred in that Fitzroy game. 5 occurred in the R1 loss to Carlton last year. 1 has never happened before, therefore by extension 6 has never happened either.
